ENTRY ASSESSMENT

The individual applying for the waiting list for the Stay Free Ministries House must complete this entry assessment form and sign it, acknowledging that he/she has read and understands the contents. If he/she gives any false information or purposely omits any requested information, the application will be denied.

Thank you for your interest in entering The Stay Free Ministry Discipleship Program. Our mission is to reach out and raise up by offering you the path to salvation, transformation, and restoration through the love and ministry of Jesus Christ. Our vision is to see lives changed, families restored, and men and women become leaders in their homes, churches, and communities. We are aiding individuals in redirecting their lives by using God's values and principles. Classes are structured to teach men and women to take responsibility for themselves, and for others while allowing God to reshape their lives.

To be added to our waiting list, the entry assessment form must be completed and returned to the SFM Director. Once received, your application will be prayerfully reviewed. You will be contacted within 3-5 days to let you know if you have been accepted into the program or placed on the waiting list. Once you are on the waiting list, we need to hear from you every week, so we know you are still interested in placement.

If you do not call us every week, we will remove your name from the waiting list.

Please keep your affairs in order. We never know when a bed may become available. When you are contacted, your bed will only be held for 24 hours.

If you are accepted into the SFM House, you will be given a rule book which you must read and sign, indicating that you are willing to submit to all the rules and leadership of SFM.
